Activity Sign-In Forms & Reports

The following are the only currently accepted Activity Sign-In Forms for EBC events: These are to be either filled out at, or ahead of, the event, and submitted in a manner that the form provides.  In all cases, the submitted form must be in the correct hands ( MemberMiles@ElmhurstBicycling.org ) within a week of the event.

Fill in and submit online.

• Guest (All non-members) Release & Waiver E-Form    Automated email submission to club Membership Coordinator). 

Fill in online and print; then scan after event and email.

• *WRITEABLE* Member & Guest Activity Sheet Combined Form, PDF format  (Can print on one, single two-sided sheet of paper. *You can print without saving, but if you want to save the data, please download this as your original and always open and save as new copy before entering data on the new document. This will preserve your personal version of the form for your future use.)

Print, fill in offline, scan after event and email.

• Member & Guest Activity Sheets Combined (2 sided), PDF format  (Can print 2 pages on one, single sheet of paper. Same as above but can only fill out by hand.)
• Member Activity Sheet (single page), PDF format
• Guest Activity Release & Waiver (single page), PDF format

• ---

• Incident / Accident Report, PDF format   Should there be an accident or other significant incident on an EBC event, the following report is to be filled out and sent to the Ride Captain ( RideCaptain@ElmhurstBicycling.org ).
